What is the adult day care admission process?
We usually suggest a tour for the family and/or their loved one. We offer two free visits so that the senior can experience our program firsthand. We provide transportation for these visits, and family members are encouraged to come along. There is a set of paperwork that needs to be completed, the most important of which is a Physician’s History and Physical form. This needs to be filled out prior to admission. To start the process or arrange a visit, contact us.
What is the cost for adult day care?
Our medical model is covered by Medicaid. Our social program is private pay at $77.00 for a full day and $57.00 for a half day. Our social program is a recipient of an EISEP grant through the Westchester County Programs and Services for the Aging. Please contact us for more information.
