The Memory Care Partner is an essential member of our Memory Care team. He or she is responsible for providing hands on care, both physical and emotional, to a specified group of residents according to each resident’s Interdisciplinary Care Plan. The Memory Care Partner assists with all Activities of Daily Living, as well as provides daily house-keeping services to the unit, creating a homelike environment. The Memory Care Partner supports the residents by assisting in meals and by facilitating activities and outings. The Memory Care Partner is also responsible for communicating with residents and their families, as well as the rest of the Interdisciplinary Care Team. Exceptional interpersonal skills, a deep desire to serve our elders, and an unlimited capacity for patience and nurturing is a must for successful performance in this position. A love of working with Seniors/those with disabilities and a passion for bringing healing and hope to the lives of residents and their loved ones is a must. Furthermore, a positive attitude and pleasant disposition is essential. MCPs must be able to work cooperatively with other nursing staff as well as staff from other services providing resident care. Dependability, accountability, and integrity are necessary for effective performance. Current NYS HHA Certification.
